Membership Fee
$160/month ages 18-39 years
$200/month ages > 40 years
Initial one-time $150 registration fee.
Membership Term
Initial commitment is 3 months and then monthly with auto withdrawal (preferred) or credit card.

Same-day physician access-
With flexible convenient methods of communication including secure text, secure email, or phone based on your preference.
Extended in-person and Telehealth visits-
With flexible scheduling when you need them.
Comprehensive preventative care-
Annual wellness exam, age-appropriate cancer screenings, smoking cessation, nutrition and healthy lifestyle counseling.
Chronic disease management-
Treatment of chronic medical conditions including but not limited to:
Hypertension
High cholesterol
Diabetes
Thyroid disorders
Asthma/COPD
Osteoporosis
Sick visits-
With point of care testing available including Strep, Flu, and Covid 19 testing, Urine dipstick, Finger stick glucose, and Fecal occult blood test.
EKG
Hospital follow-up and coordination of care with specialists.
Extra Services, for an additional cost:
House calls or nursing home visits: $50
(within 15 Miles by car/further distances can be discussed as needed)
$20-$40, depending on complexity:
Cryotherapy (precancerous skin lesion or wart removal)
Skin lesion excision
Wound care including incision and drainage
Trigger point injections
